Purpose:The purpose of these Terms of Service is to outline the rights, responsibilities, and expectations between Nak Union Behavioral Health and individuals who access or use our services. These terms are intended to ensure transparency, protect client confidentiality, promote respectful interactions, and establish guidelines for the appropriate use of our in-person and telehealth mental health services.
By defining these standards, we aim to foster a safe, professional, and ethical environment for all clients, staff, and stakeholders involved in the care and support we provide.
2. Services Provided
Nak Union Behavioral Health provides outpatient mental health counseling, telehealth services, assessments, skill-building groups, and related support services. These services are delivered by licensed professionals and support staff in accordance with state and federal regulations.
3. Eligibility
You must be at least 18 years old or have consent from a parent/legal guardian to receive services. Clients must complete all required intake and consent forms before services can begin.
4. Appointments & Cancellations
Appointments must be scheduled in advance. If you need to cancel or reschedule, please notify us at least 24 hours in advance. Repeated no-shows or late cancellations may result in a fee and/or discharge from services.
5. Telehealth Services
Telehealth services are delivered via secure, HIPAA-compliant platforms (e.g., Zoom Professional). By using telehealth services, you acknowledge and accept any limitations associated with remote care, including technology issues or limited emergency response capacity.
6. Fees and Payments
Nak Union accepts Medicaid, Care Management Organizations (CMOs), Medicare, certain private insurance plans, and self-pay. It is your responsibility to provide accurate insurance
information and to pay any applicable co-pays or uncovered costs. You will be informed of any fees prior
to the delivery of services.
7. Privacy & Confidentiality
Your privacy is extremely important to us. All information shared with Nak Union is protected in accordance with HIPAA and state confidentiality laws. Please review our Privacy Policy for full details.
8. Use of Electronic Communication (SMS, Email, etc.)
You may receive text messages, emails, or phone calls related to your care, including appointment reminders and telehealth links. Message and data rates may apply. You can opt out at any time by texting STOP. Please review our SMS Privacy Policy for more information.
9. Code of Conduct
All clients and visitors are expected to treat staff and other clients with respect. Aggressive behavior, harassment, or threats will not be tolerated and may result in termination of services.
10. Emergency Services
Nak Union Behavioral Health does not provide 24/7 crisis services. If you are experiencing an emergency or crisis, please call 911, 988, or contact a local crisis line. In addition, please seek additional counseling support from your Nak Union clinical team (i.e., therapist, peer specialist, psychiatrist, etc.)
11. Changes to Terms
We may update these Terms of Service at any time. Any changes will be posted on our website with a revised effective date. Continued use of our services after updates constitutes acceptance of the revised terms.
